The purpose of the Community Outreach Centre (COC) is to serve those experiencing homelessness or at risk of homelessness. Through comprehensive, client-centred services, we support individuals in moving toward stability and long-term self-sufficiency.

The Community Outreach Centre experienced a year of transformation
Alongside our relocation, we expanded services, strengthened partnerships, and achieved meaningful outcomes in housing, health, and employment. Our holistic approach—integrating basic needs, case management, health care, and employment supports—continues to deliver measurable results for clients and the broader community.

Housing Success
- 1 in 3 clients who accessed case management were successfully housed
- 70% in independent housing
- 30% in supportive, transitional, or recovery housing
- Long-term dependency reduced: only 40% of clients who received support in January continued to require services a year later
- Personalized housing assessments promoted stability

- Case managers supported 30–35 clients each, offering tailored plans and assessments
- 189 new referrals reflect increasing demand
